This Privacy Policy describes how and when I collect, use, and share information when contact me via my website or when I visit you for an appointment.  This is to comply with the General Data Protection Regulations (GDPR) 2018.

Toe2toe footcare is committed to ensuring that your privacy is protected.

INFORMATION I COLLECT

To ensure safe treatment, you will be required to provide certain information including:

  • your name and address
  • contact, next of kin and access details
  • medical history and information.

This information will be stored in secure premises.

WHY INFORMATION IS NEEDED AND HOW IT IS USED:

This information is required for the following reasons:

  • where it is necessary for the purposes of provision of agreed health care, in order to fulfil thorough and safe assessment and treatments, and to provide customer support;
  • to ensure mutual contact can be maintained for the duration of the agreed service arrangement.

INFORMATION SHARING AND DISCLOSURE

Sharing of personal information is limited in reason and circumstance, as follows:

  • Medical professionals.  With your consent and agreement, information may be shared with medical professionals such as your GP if further medical involvement is recommended or required, or with emergency services in the case of a medical emergency occurring during a treatment visit.
  • Specialist service providers.  On rare occasion, it may be pertinent to share relevant details of your health information with a senior specialist for the purpose of advice and guidance.  This will be discussed with you prior to any sharing, and your agreement and consent confirmed.

DATA RETENTION

A majority of your personal information would be retained only for the duration of the treatment and service arrangement, as described in my Privacy Policy. However, there is also a legal and regulatory requirement to retain health records for a time period of 7 years, to ensure availability for the purpose of future or ongoing investigations or disputes.  To ensure compliance with this requirement, essential information will be archived securely after the last appointment and all other personal details will be safely disposed of.

SECURITY

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, suitable physical, electronic and managerial procedures are in place to safeguard and secure information collected.

YOUR RIGHTS

You have a number of rights in relation to your personal information. While some of these rights apply generally, certain rights apply only in certain limited cases:

  • Access. You have the right to request access to, and receive a copy of, your personal information held about you by using the contact information below.
  • Change, restrict, delete. You may have rights to change or restrict use of your personal information. However, in the case of health records these are normally exempt from change and deletion requests.
  • Object. You can lodge an objection to (i) processing of some of your information based on legitimate interests and (ii) receiving marketing messages from me after providing your express consent to receive them.  Within the legal and regulatory requirements to which these may be bound, your preferences will be respected.
  • Complain. If you wish to raise a concern about use of your information (and without prejudice to any other rights you may have), you have the right to do so with the Information Commissioner www.ico.org.uk
